Stages of Inflammation
Ingrown toenail is classified into three stages, depending on severity:
1st-Degree Ingrown Toenail
Mild inflammation with swelling of the nail fold (paronychium), often accompanied by redness and pain upon pressure. No discharge or granuloma appears at this stage.
2nd-Degree Ingrown Toenail
Inflammation becomes more intense, causing redness, swelling, and increased pain. There may be serous or foul-smelling purulent discharge due to secondary infection, which may stain socks.
3rd-Degree Ingrown Toenail
Chronic inflammation with persistent and severe pain, along with continuous discharge. A hypertrophic granulation tissue (granuloma) forms, impeding wound healing.
Causes
Ingrown toenail (onychocryptosis) can be triggered by various contributing factors:
- Improper Footwear.Tight or poorly designed shoes compress the toes, causing trauma and inflammation.
- Incorrect Nail Cutting.Cutting nails too short or unevenly, especially at the edges, encourages ingrowth.

Treatment

If symptoms persist, antibiotics may be prescribed, especially in diabetics or immunocompromised individuals, combined with painkillers and anti-inflammatories. If conservative measures fail, surgery is required.
Nail avulsion (onychotomy) involves removing the affected nail portion. The procedure includes excision of the ingrown segment, drainage of pus if present, and, in recurrent cases, CO2 laser ablation of the nail matrix for a permanent solution.
